Forms for Volunteering, Fund Raising, Fostering for CFA

Providing the best of care and the best foster and adoption homes today requires that shelters keep excellent records not only of the animals they help, but of all the volunteers that work together to make such good care possible.  Here are the forms we use to gather information about our volunteers and foster home applicants.  Click those applicable, print them, complete them and either fax them to 620-252-6061 or mail them to Coffeyville Friends of Animals, 4955 CR 1400, Coffeyville, KS  67337.
